SUBMISSION

If possible, entries should be submitted electronically using the form provided. Participants without access to a computer can complete a printed copy of the form, which can be mailed or faxed.

Eight sections of the form must be completed, as follows:

1- Name(s) of originator(s) and their affiliations
2- Four-line biography of main originator(s)
3- Entry title
4- Format of entry
5- Language of entry
6- Description of entry
7- Delivery of entry
8- Special requirements

Details of each section are as follows:

1- A maximum of four originators is allowed per entry.

2- Enter the biography of the originator(s) in the space provided.

3- The title must be short and descriptive for Congress delegates who will refer to the program.

4- The entry may be in the form of photographs, a video recording, text, artistic work, a model, an object or other type. Information on entry format must be specific enough for organizers to display the entry, for example, if the entry is an object, specify its size and other characteristics. To handle requirements for certain items, e.g., for shipping models, a special agreement with the organizers is necessary.

5- Entries will be accepted in three languages: English, French and Spanish.

6- Each entry must be accompanied by a brief description of no more than 500 words. Items such as photos can be included to support the description. The written description of the entry may include:
1) description of the topic
2) area of achievement
3) main steps taken
4) people and partners involved
5) expected results with regard to engaging social players
6) potential contribution to the rights of people with disabilities
            and their families.

7- The sender can include the entry at the time of registration, for example, in the case of texts or other types of electronic files.

Entries in the form of objects must be sent to the following address:
Institut de réadaptation en déficience physique de Québec
a/s Claire Dumont
525, Boul. Hamel, Quebec City, Qc, Canada
G1M 2S8

The sender is responsible for shipping costs. If the object is large or fragile, or requires special handling, the sender must make arrangements with the organizers before shipping the item.

8- In the event that presenters attend the Congress, special needs such as sign language interpretation, special equipment, etc. must be requested.

Prizes

Three monetary prizes will be awarded at the Congress: $5,000 for first place; $3,000 for second place; and $1,000 for third place (in Canadian dollars). In addition, Congress registration fees will be waived for winners who wish to accept their prize in Quebec City.
